The fully functional deployment of COIL laser will be capable to destroy ICBM in a range of 600km. For the purpose of eliminating blood lipid, the target doesn't have to be at 600km, it should be about 1 to 2m, for the safety of the equipment, and operator. Also, since it is not a high speed moving target, we can take our time, say 1 to 2 minute continuous explosion. Creating air bubbles in blood should be a concern here.
